Proforma – Forensic Examination: Adult Victim of Suspected Assault/Non-Accidental Injury (NAI)

This pro forma has been developed to support the Humber Clinical Commissioning group’s pilot project looking at providing forensic medicals for adults at risk of harm who may have suffered a non-accidental injury as a result of physical abuse or neglect. The form has been designed to be compliant with the Care Act 2014 and the MCA 2005.

Members of the FFLM involved in Adult Safeguarding may find this pro forma of use in their practice.
