New FFLM Trustee Form

It is a requirement for all new FFLM Trustees to complete the form below.

You will need the following items to complete this form

Proof of address
Photo ID
Indemnity insurance
Equality and diversity training certificate

The FFLM also requires Trustees to complete an Equality and Diversity training course. If you have not already done so, please register for a free Acas Learning Online Account and complete the training.

Please upload a copy of proof of completion into the form below, or email your proof of completion to charlene.campbell@fflm.ac.uk. If you have already completed this training, please ensure you provide us with a copy of your certificate of completion.

Finally, as a Trustee your details will be added to the Charity Commission website. Please refer to the list below on information we are required to share:
