Reasonable Adjustments Request Form

This form provides the FFLM with the information needed to support you during your examination and must:

Be completed at the point of each examination application
Include supporting documentary evidence and must be dated within the last three years.

If you have a disability, health condition or study needs that may require special provision during your examination, please complete this form.

Information provided will be assessed by the FFLM’s Chief Examiner or a nominated deputy. Candidates will be informed via email if adjustments to assessment procedures will be made to meet their needs.

As defined by the Equality Act 2010, disability includes (but is not limited to) people who have:

physical or sensory impairments
specific learning difficulties (e.g. dyslexia and dyspraxia) or other cognitive impairments
mental health issues
hidden impairments (e.g. epilepsy, respiratory problems or heart conditions)
ongoing or fluctuating medical conditions, including unforeseen medical circumstance.
